I authorize The Presidents Club Barber Shop, LLC to charge Membership payments to the credit/debit card I have provided, ending in ___________, every month. By signing this agreement, I understand that I will be charged $45.00 on today's date and once a month on the 1st of each month until canceled. *Appointment Cancellation Policy: Cancel for free up to 3 hours ahead, otherwise you will be charged 50% of the service price for late cancellation or 100% for not showing up/ less than 30 minutes cancellation. Membership cancellations are to be made in the store by completing a form or by a written email request to info@presidentsbarberclub.com at least 2 weeks before your next billing date otherwise, one final payment will be processed before termination of billing is completed. Member credits accrue and don’t expire as long as you are an active member. If you cancel, you will have 90 days to use any credits you have left. The Presidents Club Barber Shop, LLC reserves the right to revise or change this agreement at any time with written notification.
